Insurance & Payment
In a
effort to keep our costs down and our fees reasonable, we ask that you pay for office
visits and other routine care at the time services are rendered, unless PRIOR
arrangements have been made. At the end of your visit, we will provide you with a
computer print-out, which contains all the required information
to allow you to file
for reimbursement from your insurance company.
In
certain circumstances, we will file your insurance for you. Please remember that
your insurance is a method of reimbursing you for fees paid to the physician
and is
not a substitute for payment. Your insurance policy is a contract between you
and
your carrier. You will be responsible for any amount not covered by insurance.
MANAGED CARE PLANS: We will file insurance for any patient belonging to a managed
care plan with which Dr. Hallett has contracted to be a preferred provider. You will
be required to pay your co-pay, deductible and/or percentage amounts.
MEDICARE: We are currently NOT accepting any NEW Medicare patients into our
practice. However, current patients who become Medicare
eligible will continue to be seen. We file all Medicare insurance for our patients. However, we
do not accept assignment, therefore you will be responsible for paying for
services at the time they are rendered. Medicare and any secondary insurance will
reimburse you.
Our
office staff is always available to assist you with any billing or insurance
questions that you may have. For your convenience, we accept Cash, Checks, MasterCard,
Visa and Discover.
